There is a wealth of resources available for patients with Chronic Kidney Disease. The CKD Insider Newsletter and the One Track Health mobile application developed by Dr. Ann Chokas, were born out of a desire to share knowledge to help patients and their caregivers to quickly access important health information and for disease management.     Please know that the free OneTrackHealth app to help people with chronic kidney disease (CKD) on dialysis is now available on all platforms (apple, android and web – for people without a smartphone). The free app allows users to view, track & share test results and stay up to date on the latest CKD news and educational information. The app is fully customizable so you can track the tests that are important to you and/or your loved one. You can learn more about the app and download it here.  

    Please share this information with anyone you think might benefit from using the app, including care partners. My goal is to have the app adopted by as many people as possible and get their feedback to further improve the app.

    The OneTrackHealth app is designed to be a simple personal companion for people with chronic kidney disease (CKD) who are transitioning to dialysis, are on dialysis or have had a kidney transplant and their care partners. The app is customizable for anyone tracking results.  

    The OneTrackHealth app features that differentiate it from My chart are below:

    1.    Simple 2 color coded view of test results located on the dashboard makes it easy to see if you are on track (within test reference ranges) or off-track (outside of test reference ranges) with your test results.

    2.    5 test types are included with the recommended test reference ranges for a person on dialysis. Test result ranges were determined by consultant nephrologist. The blood test types included are: Albumin, Calcium, Hemoglobin, Phosphorus & Potassium.

    3.    Test Help Guide: Provides information about each of the 5 test types and the test result reference ranges used in the app with the supporting published medical information.

    4.    Customizable features:

    ·     Add or edit test types and test result reference ranges as you want with same 2 color coded view. Consultation with your physician recommended.

    ·     Add as many people (profiles) & test types as you like.

    5.    Individualized sharing capabilities:

    ·     You can share the specific information you want from the app versus sharing your whole medical history.

    ·     You can share information you have entered from outside your health system, state or country.

    ·     Benefit of sharing with a care partner via the app: Your care partner will not only see the same test result information, but will also learn with you and stay up to date on the latest CKD news and educational information to support you.

    6.    Chronic Kidney Disease (CKD) News Section includes:

    ·     The CKD Insider Newsletter: Provides quick to the point actionable information from kidney organizations including TransplantLyfe) about upcoming CKD events and educational series, new FDA approved medications, clinical trials, and other support related information. The newsletter also includes a Fun Tip of the Day!

    ·     Educational content for people on dialysis written by dialysis care team members.   

    7.    Data input: Test results are currently typed by the user into the OneTrackHealth app. Direct importing of test results is offered as an option for kidney providers. If your healthcare provider is interested in this option please have them email info@onetrackhealth.com.
